Clay Primary & Family Care Center

All of our healthcare providers are currently accepting new patients.  We accept most insurances, and Self Pay patients are always welcome. Please be sure to bring your insurance information and a photo ID to your first appointment.  We ask that you arrive at least 20 minutes prior to your appointment time to allow for the necessary paperwork.  Walk-in patients are always welcome.
